It mean two  or more notes played together that don't match. , in other words, don't sound good together. ......are not a real chord.

It's what mycatsmom said, and the thing is, like everyone else is saying....it is often intentional.

Using the piano as an example:  striking together C, E, and G will sound pleasant, as will striking together C, E-flat, and G.
Striking together C, E, and F# does NOT sound nearly so nice. That is an example of discorance.
