You pretty much know the basics:  Born on  (month, day, year) in (city, state, nation) to (mom, dad).  (Birth order) of (how many kids).  Brother?Sister? of (siblings' names).

Some interesting things about yourself as a baby (ask your folks).  When you got your first tooth, started walking, talking, rode a tricycle, threw a ball.  

Things you like to do now and how you got started:  ie:  began piano lessons at age 6, with (Name of Teacher). Have performed (WHERE, WHY, ANY AWARDS?)   Soccer player since age (?).  What do you especially like about what you do (Music is how I erase the dirt of the day.  Soccer gives me a chance to get great exercise, be with friends, and enjoy a competitive team sport. )

Goals you have for yourself.  Plans you have for today, tomorrow.  Obstacles you want to overcome.
