To get a replacement card:

Complete an Application For A Social Security Card (Form SS-5); and
Show us documents proving:
Identity; and
U.S. citizenship (if you have not already established your citizenship with us), or immigration status if you are not a U.S. citizen.
All documents submitted must be either originals or copies certified by the issuing agency. We cannot accept photocopies or notarized copies of documents.

Get a certified copy of a document showing a birth, marriage or divorce that took place in the U.S.

Take or mail your completed application and documents to your local Social Security office. Any documents you mail to us will be returned to you.
