In the USA you don't need anything for a small one person business unless it is one that requires a license (such as food preparation etc.). Keep good records and enter into tax return when you pay taxes.
If you want to set-up a business bank account, you will need to go to your county and apply for a DBA (doing business as) account.
Anything on a larger scale, I recommend an attorney or possibly CPA. Good record keeping is a must!
