You can download the form and send in I think a money order for about $5.00 or something close to that and get a printout sent back to you from the DMV in your state.

Here is an easier and quicker way. If he is on your driving insurance policy call your agent and ask him to look up his DMV record. If he is not on your driving insurance policy call your insurance agent and tell him you want to add him to your car insurance policy and want a quote. First thing he will do is run his DMV driving record and call you with a quote. Then ask him why it is that much and how many traffic violations show on his DMV record. You might need his DL # if he is not currently on your insurance policy though.
