Your medical professional is responsible for prescribing medications. YOU are responsible for knowing if you are allergic to the medication, and for asking intelligent questions about the med before accepting the prescription.
Your medical professional may be a general practitioner, register nurse, physician's assistant, specialized "doctor", to name a few.
Your pharmacist should be cross checking all your prescribed medications before dispensing one that may have a negative interaction with one which you are currently taking. But they don't.
