Unless there is a medical reason for the obesity, then yes, I feel it should be labeled abuse if the obesity is due to an improper diet that causes the child to gain weight to a level that can lead to health risks or even death. Obesity also carries the threat of metal abuse if the child's peers target and make fun of him/her on a daily basis. Undernourishment is considered abuse so over nourishment to the point of endangering the child's life should fall under the same heading of being abusive.
