    Have the rules changed regarding use of "couple of leaves, pages, etc"?

    Your teacher can best answer this. He/she has to continue their own education to maintain their teaching license. I know what I was taught in schools up to the year I graduated (1980). If there have been changes since then, the teachers would know.

    I was specifically referring to recent use of "couple bandits held up the bank" in a novel. Until recently it would have been "a couple of bandits held up the bank". Your comment please, Colleen.

    It should still be "a couple of". I'd say this is a print error. Many books have errors in them. Even editors and proof readers miss things.
