It is a term that is used when a person does not bathe enough.  An accumulation of dirt, if you will, between toes. 

... Please cleanse regularly.

Back when I was in the service, we would be out in the field for weeks at a time, we collected toe jam regularly, pretty tasty on toast with light butter................ Ok  kidding. Probably one of the most important things that was stressed to us, being In An infantry unit was to take care of your feet.
