    You have more than one client (clients), so you have their families because they all do not share one family.  You don't have a complete sentence here, though.....what about the plans and assessments?  Are they available at no charge?  Are they unavailable until May?  

    Plans and assessments are unique to clients and their families.


    The above sentence is complete, your sentence was not a  full sentence only part of one.
