A birth certificate is issued directly by a hospital with the information provided by the attending physician and/or nurse. The birth certificate is sent to the city/town clerk where the hospital is located. A Certificate of (Live) Birth can be obtained by the parents for use in enrolling the child in school, obtaining a social security number, getting a job etc. The original birth certificate is kept by the city/town clerk and a copy of it will be sent to the Vital Statistics Office of the State in which the city/town is located in.
A Certificate of Birth is issued by the hospital. A Birth Certificate is issued by the county.

This may vary from state to state, this is how i find it in Ohio.
