What vets do is put on a glove, lubricate it, reach in and gently press against the glands. The secretions are strong smelling. (I can't honestly say I'd have the stomach for a task such as this.) But, that's the bottom line of what you are talking about. When a dog doesn't have worms, those glands needing emptying is usually why you'll see them drag their butt, or spin around with their buttt on the ground.
