    Start the letter Dear Sir/Madam,

    Further - "I should be most grateful if you would", never say "if you could" - mostly people can, but will they.

    Madam generally precedes Sir as you have it

    If you wish to address someone with 'dear', the word dear should be said or written before sir or madam.
