The only treatment with potential for cure from pancreatic cancer is surgery.

If the tumour is resectable, then the treatment of choice for a pancreatic tumour located in the head or neck of the pancreas is a pancreatoduodenectomy. When the tumour is located in the tail of the pancreas, a distal pancreatectomy is recommended. In addition to curative-intent surgery, chemotherapy, and in some cases also radiotherapy, may improve survival. Such adjuvant treatment should be given within 6 weeks after surgery.

If the disease is non-resectable, i.e. at an advanced stage, then the aim of treatment is prolongation of life and relief of symptoms. Although the disease may not longer be cured, palliative treatment with chemotherapy, radiotherapy, surgery, or other medical procedures (e.g., treatment with biliary stents that ease passage of bile if the tumour obstructs the bile ducts) may be effective palliative treatments for pancreatic cancer.
