Physicians are medical doctors. Yes, they have to be in order to practice. It means they went to school for this (for most of their lives) and are trained and able, they have the knowledge.
There are doctors of philosophy etc. and they have a higher learning degree in their theoretical field. They didn't go to medical school (therefore, no M in their title).
