        This is far too literal and superficial - on another planet would basically mean that you have a different opinion.

        It means that they do not know what they are talking about. Mad in it's truest sense is somebody who is mentally ill i.e. psychotic people who murder others insane involves many severe illnesses. People use these words so flippantly - sometimes jokingly which the receiver realises but when said seriously it is because of ignorance, judgemental and abusive.
