First of all, never keep a birth certificate or a Social Security card in your wallet. Someone else is able to use your identify if they get their hands on them. Leave them at home, do not carry them around with you.

Get a copy of your birth certificate from Vital Statistics in your state. Look in the Government listings in the phone book. Next, check with the local Social Security office in your state and find out what to do to replace your SS card.
