If you're still working it is recommended that you stay insured with your employers insurance policy (if there is one) and use the medicare as a supplemental to it. If there is no employer insurance policy you will be paying for the medicare out of your own pocket or your paycheck.
You don't HAVE to go on Medicare at 65, only if you are retired, most employer contracted insurance companies don't insure people who are retired and 65 years old.
