          Your mother's maiden name means her surname (last name) before she married.  What was her name in first grade, basically.


          For example.  Bob and Jane Smith have a daughter and name her Janice Marie Smith.  Janice grows up and married Don Ward.  Now she is Janice Ward.  Her MAIDEN NAME is SMITH.
