I can only assume the app is on your PC.
In order to uninstall a program press the 'Start' button, then open the 'Control Panel' and double click the option 'Programs and Properties' or "add/uninstall programs" then search for the program you wish to uninstall on the programs list, double click it and choose 'Uninstall'.
If you do not see it in your programs, go to Tools on your browser, look for add ons or extensions, click it, look through the list, find the one that is annoying you, check mark it and either send it to the trash or disable it.
Instructions for a cellphone depend on the cellphone you have.
