I think it means that our health is affected by what goes on around us. The pollution or cleanliness of the air and water, the amount of crime, the ethnic makeup of your community, the population (how large, how congested); your personal situation (happy at work, sufficient income for expenses, family relations positive, spouse or single, kids and their dramas and dilemmas, successes). All these things are "social" and those things DO have an affect on your health.
