on any of your paperwork sent along with your original claim acceptance there should be some sort of web site thaqt would direct you to the company that is oaying your diability, if it is being paid by private insurance that is part of the benifits from your employer. If it is government disability payments you should be able to look up online or call the bvranch you are recieving from in order to make a formal request for the information you are seeking. It is possible that the government my require a written formal letter in order process your information.. 1st thing 1st however find out the payee of your benifits and the rest is just asking the questions to the person on the other end
