The illness would be called "terminal" - meaning it will end the life of the patient.
The patient's giving up hope could be called "resigned".
The "prognosis", or expected outcome, is "dire", "serious", "untreatable".
The doctor telling the patient and/or family/loved ones needs to be compassionate and humane. They aren't always, but don't get me started.
