    Mrs mary is in lot of discomfort and is continuall caring in bad the doctor has been notified review her current pain management. however , the daughter is in room and is getting extermely agitated which in turn is disttressing mary you have suggested to her daugher that she has a break but she refuses to leave the room

    You must do what is right for the patient, if she is distressed,  ask the daughter to leave the room until her mom settles down and is comfortable. This must be upsetting for the daughter to see her mom like this, sounds like the daughter could use some words of comfort also.

    Mrs Mary, should be transferred to hospital, where she can have 24 hour nursing care, This way, both Mrs Mary, and her daughter, will have less stress.
