    Contact the bank or lending institute and check with the loan officer. They can answer this best. 

    Usually no modifications take effect until there are concluded. It makes sense to finalize any change by post-dating it to the date of the next due payment to simplify the transaction but as Colleen correctly suggested, speak to your mortgage lender and tell them that's the way you'd prefer to do it. Assuming,of course, that you don't have a different preferred method.
