      The doctor does and it's for recording notes of treatment. exams, vaccines, etc  that the doctor has given the patient.

        Medical record is also kept on record,  for information in future if required. Example, if one of your children became ill. maybe even gran children, and Dr. not sure what is wrong, he can check back in medical notes, and see did any of previous family have some illness.that is characteristic.
