      It seems to be a 19C English expression. I suspect it has something to do with the Khyber Pass, the entrance to Afghanistan from what was India. Soldiers going into the pass were unlikely to come through in one peice. It is clearly a 'slang' word.

        Interesting question. If you visit www.worldwidewords.com you will see a discussion about the etymology of this word, which comes to the conclusion that its origin is unknown. Perhaps one of the clever people who use this forum can make some suggestions.
