As anything that you wish to become a legal document, you need to include certain things.  The description of the item (car) year, make and model, and vin number (check the actual number on the car, to insure that it matches with the title vin number.  You should also note the mileage on the odometer.  To be sold to (your name) for the sum of $___by the owner (the sellers' name) on this date (date of sale).  This sale in with no guarantees or warrantees, and is an "As-is" sale.  remember to have the actual owner sign over the title to you, and get the title, as you must take it to the registry with the bill of sale. Have both parties (buyer and seller) sign.
