In order to get outside of the country you are at you need to cross its border, which can be done by flying an airplane to wherever, or by going to you country's border and crossing it to one of the neighboring countries, which sometimes require acquiring a visa at the respective embassy.
If you live in an undemocratic country, such as Iran and Syria, it might be more complicated.
