There are many types of spa treatments.  Give her a birthday gift that keeps on giving by arranging for her to be treated to different type spa treatment once a month on the day of her birth.

My wife loves facials, they rejuvenate her and are very affordable.  She has a favorite technician, Eff Eff, and it is a real treat for her, from her hectic life as a grandma. 

Pamper her, like CB said, with certificates for different treatments throughout the year.  Facials, waxes, manicures, pedicures, massages, so much from which to choose.  If I had to pick just one, I'd go for a simultaneous massage and skin treatment
