Pour the paint through a strainer. A strainer with very fine holes is ideal because it will not let large, medium or small chunks through until the chunks break down. Pour the paint back into the can and repeat the process to break down the chunks and leave it smoother. Pour the paint back into the can.
Stir the paint again to mix it well and find any remaining chunks or move them to the bottom of the can. Check the paint by brushing it onto the newspaper. Repeat as often as necessary.

Probably simpler to junk the old paint and buy new.
