It's isn't necessary to respond to every individual's reaction. ACKNOWLEDGE, certainly, out of courtesy, but not necessary to respond to every eye roll, sigh, or "tsk", nor every round of applause.
If you must respond, ask for clarification to the eye roll, sigh, "tsk", or applause.

to maintain clear understanding, and to positively acknowledge that you are interested in what message they are trying to convey, also to acknowledge their feelings whether they are positive or not, communication may not necessarily be verbal and its important to observe signs of agitation, happy, sad etc, paraphrasing is an helpful tool
