A quick and effective way to free-up your shower head is to immerse the head in a bath of vinegar.    

If you heat up the vinegar it will clean the shower head much faster.

If you're lazy, just use a toothpick to unclog debris.

CLR, will dissolve the lime deposits. The only way to prevent this probably would be to get a water softener in your home but they can be very expensive.

get a ziplock bag and put vinegar in it and put shower head in bag . Citric acid works well also. Set in solution for 6 hours , overnight better.

You can buy this product called Limeaway in the cleanser section and put some in a container and if you cannot take your shower head off and soak it in it then you will have to stand there with the container filled with Limeaway and soak it that should work.
