Word 2010 Table of Contents - Headings/Page Numbering is Off

I'm trying to create a table of contents and the numbering is off.  I've been using the proper headings functions, but the table of contents creates sections that don't exist. How do I fix it so that the proper headings are represented with the proper page numbers? In the example below, I used heading 1 and heading 3, so why does it include 1-6 (which doesn't exist) and then finally goes to 6.1?

    1. Incentives (RFP Section 6.1)........................17

    1. ......................................................................17

    2. ......................................................................17

    3. ......................................................................17

    4. ......................................................................17

    5. ......................................................................17

    6. ......................................................................17

    6.1. ...................................................................17
