I believe this can pose some very complex issues, every individual has the right to confidentiality, and I try to maintain enough respect to comply with this, even though some things at times are a bit hair raising, the only time I may consider breaking this is there is a for seeable risk either to that person or to others

And in the case of children or teenagers, quite often the potential tension is just somebody getting P.O.d because "you told" or "you didn't tell".

OK. My Brother does not speak to my Mother. I know that my brother is thinking about divorcing his wife. He has confided in me and I think my mother has a need to know. Do I disclose my concerns or maintain my brothers confidentiality. He has not told me not to tell. That is just one example.
Or the confidentiality between doctor and patient. Do you disclose to that patient's wife that he has terminal cancer out of concern for her or do you not tell because of confidentiality of the patient?

Medical information is strictly between the staff (DR.s office/hospital/etc ) and the patient you CAN NOT TELL a Spouse or Family member ANYTHING that the patient wants to keep private regardless of a fatal diagnosis or something potentially dangerous to the spouse such as AIDS. Period.
