My employer physician did a history and physical for observation of a patient in the hospital and used the codes 99218 and 99217. Medicare denied the place of service and the hospital setting is a code 21. What place of service matches the codes given. Also what code do we use for the H&P in this case??
Riverside County/Ca under North Ca jurisdiction. Medicare help line is no help.l
